Job Description

The Program Manager provides support, assistance, and guidance to individuals with disabilities that will enable the individual to develop skills and enhance their ability to engage with their community. This position is responsible for overseeing individual services to consumers receiving Day Support services.

The Program Manager is responsible for supervising program services staff and facilitating compliance with regulations/standards for DMAS, DBHDS, CARF, DOL, HIPAA, contractual requirements, and federal, state, and local laws and regulations. This position promotes best practice services consistent with the philosophy of social role valorization. This position oversees all assessments and plans development for long-term support in Day Support. This position performs rehabilitation, counseling, case management, and support for assigned consumers; develops, implements, and documents individual service plans.

  • Conduct intake, assessment, and long-term support planning for all Day Support consumers.
  • Coordinate, lead, facilitate, and assist in the development of the Individual Service Plans (ISP) following Person-Centered Planning (PCP) practices, DMAS, and DBHDS regulations.
  • Writes Individualized Service Plans (ISP) in accordance with Didlake policies and procedures, oversees the completion of Quarterly reports, and daily/weekly documentation of individual progress.
  • Deliver Day Support services that are in full compliance with Home and Community-Based Services (HCBS) Settings Regulations;
    • In a setting that is integrated into and supports full access to the greater community;
    • Is selected by the individual from a variety of settings options;
    • Ensures individual rights of privacy, dignity, respect, and freedom from coercion and restraint;
    • Optimizes autonomy and independence in making life choices;
    • Facilitates choice regarding services and who provides them.
    • Requires that the person-centered planning process is directed by the individual or individual's chosen representative;
    • Outlines the minimum requirements for person-centered service plans with individually identified goals and preferences.
  • Addresses a holistic spectrum of the consumer's life including but not limited to employment, medical, housing, psychiatric/psychological, behavior management, family relationships, education, finances, legal issues, daily living skills, assistive technology, specialized therapies, and/or social/recreation.
  • Educates consumers regarding services/employment options that reflect consumer choice as a first priority. Encourages and educates consumers on the benefits of choices that maximize community integration.
  • Coordinates, leads and facilitates interdisciplinary team meetings for consumers receiving services and supports.
  • Provides crisis intervention.
  • Collects, writes and maintains data and appropriate documentation in accordance with DMAS, DBHDS, CARF, DOL, and HIPAA accreditation standards, and assures full compliance with Didlake, Inc. policies and procedures.
  • Regularly audits consumer records and documentation in accordance with Didlake policy, referral source requirements, and standards promulgated by the Department of Behavioral Health and Development Services-DBHDS, Department of Medical Assistance-DMAS, Commission on Accreditation of Rehabilitation Facilities-CARF, DOL, and other governing agencies.
  • Oversees the daily operations of the program including managing consumer activity schedules and staff assignments.
  • Works collaboratively and maintains regular contact with all members of the consumer's support team including family members, residential providers, CSB Support Coordinators, and Physicians (if applicable).
  • Provide direct support to consumers as needed including group training, community outings, assistance with personal care, feeding, etc.
  • Links identified services and supports needed to maximize the successful achievement of outcomes outlined in the service plan to include assistive technologies and identify any environmental modifications needed.
  • Maintains employee's work time and consumer service time in eTime in order to ensure accurate attendance for billing.
  • Provides a continuous review of consumer progress toward established outcomes and service coordination through:
    • regular review of narrative and assessment data;
    • regular review of progress with direct care staff;
    • on-site observation of consumer participation and behavior; and
    • interaction with consumer, family, and other people who serve as a natural support.
  • Coordinates authorizations from funding agencies and secures approval before service implementation.
  • Provide management, training, and support to staff and consumers.
  • Completes the assessment, intake, referral, and discharge process.
  • Assesses the value and interprets clinical reports from physicians and clinicians. Ensures shared information about the individual assessment and planning information to appropriate Didlake employees designated as direct care staff.
  • Oversees the implementation of specialized training and support in accordance with service plans and activity schedules for consumers receiving Day Support services. (Activities of daily living, money management, social/recreational activities, community integration, etc.)
  • Promotes consumer rights and responsibilities and serves as an advocate for all consumers who receive services from Didlake, Inc.
  • Serves as the contact for information and referral to community representatives.
  • Assists the Director with monitoring the proper utilization of Medicaid, and CSB funding.
  • Provides reports and data as needed to meet departmental and interdepartmental requirements.
  • Communicates employee and consumer-related issues to the Director of Community Services or other designated management.
  • Compiles all program evaluation data and outcome measurements on a regular basis.
  • Participates in corporate and community committees as assigned.
  • Facilitates and fosters teamwork with other Program Services Specialists, Program Services Assistants, and other employees within the corporation.
  • Coordinates with Human Resources on recruitment, staffing, employee relations, and performance evaluations for employees
  • Participates in fiscal year budget development and implements approved budgets.
  • Attends staff meetings with supervisors and departmental staff.
  • Completes and maintains all required Didlake training.
  • Follow safety protocols to include wearing designated personal protective equipment.
  • Thoroughly clean and disinfect assigned program sites and vehicles at least daily and regularly disinfect areas designated as "high touch" throughout the day.
