Overview

The Strelitz Diabetes Center, Endocrine and Metabolic Disorders Division, Department of Medicine is recruiting for an Advanced Practice Provider (APP), a nurse practitioner (NP) or a physician assistant (PA), who will function as a clinical provider. The APP will participate in all aspects of patient care and will participate in the supporting roles of consultant, and educator. The main role of the position will include outpatient care of patients as well as in-patient consulting and conducting hospital inpatient rounds. Primary focus will be on patients with diabetes mellitus, but may also include other endocrine disorders. This position will spend up to 50% of their time covering hospital call and rounding in the inpatient setting.

Responsibilities
    Independently assess, diagnose, & develop treatment plans for patients in the area of Diabetes and Metabolic Disorders in accordance with protocols and standards of care.

  • Elicits and records a comprehensive and accurate past medical and social history and review of systems for diabetic patients.

  • Performs appropriate physical exams and health maintenance, to establish a treatment plan and follow-up care including any need for referrals, physician intervention, or hospitalization.

  • Differentiates between normal and abnormal findings and orders appropriate diagnostic studies.

  • Prescribe, dispense or deliver medications under the practice agreement.

  • Assist with hospital in-patient rounding.

  • Based on departmental needs, participation in the office call schedules including call coverage for office patients after hours on weekdays and weekends.

  • Maintains effective communication with physicians and providers regarding individual patient care to develop a coordinated comprehensive care plan on cases involving complex medical or psychosocial issues.

  • Assess learning needs, readiness, and capabilities of individuals and families to tailor education to meet the needs of the individual.

  • Counsels with patients in regards to their health maintenance, disease prevention, and medication adjustments/additions.

  • Maintains strict confidentiality of patient information in accordance with HIPAA and handles all matters professionally with sensitivity to racial, ethnic, and sexual orientation issues.

  • Maintain effective working relationships with faculty, staff, residents, patients and the public.

  • Participate in development and implementation of divisional and departmental quality assessment/improvement/research programs and processes as requested.

  • Contribute toward the achievement of divisional and departmental goals.

  • Participate and contribute in/to activities that enhance clinical expertise and nursing knowledge.

  • Participate in professional nursing activities in the division, community and professional organizations as needed or requested.

  • Performs other duties as assigned
Qualifications

  • Nurse Practitioner must have MSN and must be a licensed RN by the Virginia Board of Nursing and a certified Nurse Practitioner.
  • Physician Assistant must have an MPA, and must be a licensed Physician Assistant within the Commonwealth of Virginia.

EXPERIENCE PREFFERRED:

  • Three years of experience as an APP is preferred.
  • Experience in taking care of diabetes patients or having a certification in diabetes education.
  • Must have ability to function independently, as well as, be a member of our health care team to deliver quality care to our diabetic patient population.
